Completion Acknowledgment

By signing below, I acknowledge and assert that I have contracted for the provision of labor and/or products or equipment from Practice Sports, Inc. (“PSI”), and/or PSI’s subsidiaries, affiliates, and/or sub-contractors (“PSI Entities”), and all labor and/or products provided by PSI and/or PSI Entities are to my satisfaction, I have no disputes with the service performed or products and equipment provided up to the date listed below, the labor and products have been provided in accordance with all agreements between me and PSI, including without limitation, the Payment and Shipping Terms, available at Payment & Shipping Terms, PSI has fully satisfied its obligations under any agreement between me and PSI, and I agree to pay any outstanding balance to PSI (stated on invoice sent via email) in full.

My signature below represents my acknowledgement and consent of my obligation to provide payment to PSI in full through any means reasonably acceptable to PSI (bank account draft, credit card etc.). I understand if full payment is not remitted on the day of delivery of the products or completion of installation services, that PSI will retain ownership to the products and equipment provided, and I may incur a 3% finance charge (accrued monthly) and a late fee until payment in full (including any accrued finance charge and late fee) is made. I also understand my signature does not nullify any applicable warranty outlined at Warranties.

Under no circumstances shall PSI or any PSI Entities be responsible or liable for installation, reinstallation, or relocation of any equipment or products manufactured by me or any third-party.

I acknowledge that I am fully responsible for compliance with applicable laws, including local and municipal building and fire codes and requirements. I acknowledge that I bear full responsibility in regard to positioning of the products and equipment, including compliance with any applicable law.

Under no circumstances shall PSI or any PSI Entities be responsible or liable for damages or injuries sustained as a result of the use of PSI’s products or equipment. I have the sole and exclusive responsibility for inspection and maintenance of all installed products prior to each use, including the first use. PSI recommends regular inspections of any products or equipment. I acknowledge and assert that, in the event that the services and/or products and equipment were provided to an organization or entity, I have the full power and authority to represent and bind such organization and effectively execute this Acknowledgement.
